All forms must be turned in on the first practice date. Failure to do this will exclude the athlete from participating in team activities until forms are completed.

• Every student athlete must have an Emergency Medical Authorization (EMA) form on file with the athletics department by first day of practice.  Please complete the electronic EMA beginning July 1st, 2011 at our secure site, here.

• Butler Fitness Center Waiver (required for use of fitness center)
• Hall of Fame Nomination Form
• OHSAA Physical Form (All 7-12 grade Athletes)
• Physical Education Alternative Activity Form
• Transportation Release Form
