| ALL Campers and Students: |
| Standard medical information is collected in the registration process. If your child has asthma, serious allergies, diabetes or will be taking a prescription medication at camp, additional information is required and is found in the list below. All medical forms are due two weeks prior to the start date of camp/class. |
