Nurse's Office Forms

Below you will find a description of all medical forms that may be needed during the school year.

To access these forms, please log in to Laurel's NetCommunity.

Prescription Medication Authorization Form:  Required for all students taking prescription medication here at school (also including Epi-pens*, inhalers* etc.)  Requires a physician’s and parent signature.  For the student to self administer an inhaler or Epi-pen requires physician approval.

Non Prescription Medication Form: For medications not available on the Laurel Over the Counter form.  Use for any other non-prescription drugs that need to be taken (ex. Tums). Completed by parent.

Asthma Management Care Plan Needed for all students who use an inhaler for any reason including sports.  Please provide detailed information of symptoms, reactions and treatments for proper managed care while at school.

Serious Allergy Care Plan: Needed if an EpiPen (Epinephrine Autoinjector) is prescribed. Please provide detailed information of symptoms, reactions and treatments for proper managed care while at school.

Ohio Department of Health Immunization Report: All students in Ohio are required to have up-to-date immunizations documented and on file. Form available from the nurse’s office or admissions office.
Pre-Primary students a signed and dated immunization record is required for all students on a yearly basis.

Physician's Initial Health Report: Required for all students NEW to Laurel at the time of admission and submitted before the first day of class.  Form is available from the nurse’s office or the admissions office.
Pre-Primary students this must be submitted every year prior to the first day of classes.  This exam must have taken place within the past 12 months and subsequently then submitted within one month of the next yearly physical.

Athletics: Any athlete using an inhaler is required to complete a Prescription Medication authorization form as well as an Asthma Management Care Plan.  All other OHSAA forms can be found on the Athletics on website.

Emergency Medical Authorization EMA: Completed on NetCommunity prior to the start of the school year. This lists the primary contact information, medical and health related concerns and medications.

Over the Counter Permission Form:  Completed on NetCommunity prior to the start of the school year. Medications provided by Laurel with standing orders from the Laurel house physician.
